Consultation Services
We partner with family members and friends to provide step-by-step strategies to
manage activities of daily living and address medical, legal, and financial concerns. We
help our clients navigate eldercare with confidence and direction. We provide services at affordable rates. Our services include:
1. Assessments
- Review of the senior adult’s current living arrangements
- Assessment of activities of daily living and support services
- Review of in-home safety and elements of preventive care
- Assessment of personal care services and coordination of required services
2. Development of a Customized Plan to address:
- Short-term and long-term care management
- Completion of Medical Directives
End-of-life Arrangements - Relocation to appropriate housing – Assisted Living facilities, Independent Living
- facilities, Skilled Nursing facilities
3. Healthcare Coordination
- Discharge Planning
- Coordination of provider services
- Scheduling medical appointments and accompanying the senior adult
- Coordination of the delivery of prescriptions, medical supplies, and equipment
- Coordination of Respite Care
4. Resource Navigation and Referrals
- Vetting professionals and providing referrals, including to Home Care Agencies,
- Elder law Attorneys and Nutritionists
Accessing appropriate Assistive Devices – beds, wheelchairs, walkers - Finding stimulating, appropriate social activities
- Accessing safe, reliable transportation
